Integrated Health Home Program (IHH)

 Winter 2020 Newsletter

The Integrated Health Home Program is an approach to healthcare that partners Families First Counseling Services with you in caring for your child. This FREE service is an expansion of your existing Medicaid benefits.
We are a team consisting of a care coordinator, family support specialist, and a registered nurse that is committed to helping coordinate services with doctors, behavioral health practitioners, schools and others to make sure that all of your child’s emotional, mental, and physical health needs are being addressed.  The Family Support Specialist is a unique position to the Integrated Health Home program that can best be explained by a video created by the Iowa Peer and Family Support Training Program.  Click here to watch the video.
Your team will help your family make and reach their goals. They will not replace existing providers, but are an additional support to help pull it all together. You do not have to change your current services.

What are the benefits of the Integrated Health Home Program for my child?
